Complex Assessment & Liaison Team (CALS Team)

North Bristol Trust places a high priority on ‘Frailty at the Front Door’. This team provides detailed assessment of patients that have complex care needs at the point of their admission, facilitating rehabilitation, early discharge planning and speeding care transfer.

This is an exciting time to be joining us as North Bristol Trust has just joined the Acute Frailty Network to ensure the multidisciplinary Complex Assessment and Liaison service is capturing frail, complex patients at the earliest opportunity and providing our patients and staff with state of the art facilities in which to deliver exceptional healthcare.

We are looking for a dynamic practitioner to join the team for a fixed term contract to cover maternity leave. We pride ourselves on excellent communication both within the team and the wider health community.

The post holder will work closely with the established physiotherapists, Consultants, Advanced Nurse Practitioners, Occupational therapists, Case Managers and social care partners to ensure early information gathering, advising and facilitating moves to alternative care services and enabling early treatment and care planning to facilitate prompt discharge.

We encourage our therapists to develop their skills and have an active programme of peer review as well as formal multidisciplinary teaching.

We are delighted to have received consistently positive feedback from patients and their carers, and have also significantly reduced length of stay within the Trust. We work closely with our community teams and are the hub through which new initiatives are implemented, for example, Discharge To Assess and care planning for the frail elderly.
