The Frailty Team at North Bristol NHS Trust are looking to recruit an Advanced Nurse Practitioner Band 7 (Band 6 depending on experience & qualification).
The Frailty Team enables early Complex Geriatric Assessment (CGA) for those who need it within a few hours of presentation in the Emergency Zone. The team consists of Elderly Care Consultants, Advanced Nurse Practitioners, Physiotherapists and Occupational Therapists. The team optimise treatment pathways for complex frail patients on the AFU (Acute Frailty Unit) who see direct admissions from GP’s and the Emergency Department, within the Complex Care Wards and the Elgar enablement Unit.
The team provides early rehabilitation plans and ensures appropriate patients are supported by case management. They work in collaboration with social services and have allocated support from South Gloucestershire and Bristol social workers.
In addition they provide support and advice for ward based teams in planning care for complex patients. The Frailty team operates a 7 day service from 8am – 6pm.
It is envisioned that the post holder will be able to communicate between hospital and community care settings and work closely with the lead consultant, AFU and AMU teams and social care partners. The team ensure early information gathering, advising and facilitating moves to alternative care services and enabling early treatment and care planning to facilitate prompt discharge.
The team delivers known benefits such as reducing length of stay and preventing unnecessary hospital admissions.
You must have appropriate UK professional registration.
