NBT in partnership with commissioners, community health and social care teams has transformed in the last 5 years its integrated discharge service (IDS) and we are looking for a passionate leader with community experience to help us continue our improvement journey.


We invite applications from candidates with recent experience of managing services in the community and ideally experience of also working in a large Acute Trust. Candidates with lesser or no acute experience would be offered an 8B development role, whilst those who can effectively demonstrate suitable experience in both acute and community care and meet all requirements of the person specification would be appointed at 8C level.


The post holder will play a key role within the Trust in the development and delivery of high quality clinical services, co-ordinating effective day to day performance management across an agreed portfolio of services and will lead a portfolio of transformation and improvement programmes. They will effectively diagnose areas for further improvement and ensure service monitoring contributes to the wider performance and assurance framework for the Trust.


This role requires an individual who is driven, ambitious and passionate about patient care, and who has a desire to influence at the highest levels working in close partnership with senior clinical and managerial staff within the Trust and in partner organisations.


The post holder has a clear mandate to ensure they develop NBT IDS services within the Trust; both in terms of complex discharge and in support of admission avoidance and High Impact User plans to reduce LOS.


This role will be managerially responsible for the NBT IDS team, consisting of two B7’s Lead Case Managers and a multidisciplinary team of B6 case managers.
