Acute Trust Case Managers

37.5 hours per week ( part time considered) inc. some weekend and BH working.

Are you passionate about ensuring a safe and timely hospital discharge for your patients? Are you an experienced  registered health or social care professional with knowledge of  this area and an interest in service improvement?  Are you a team player but confident with managing an individual caseload?  If so then we are interested in you!

NBT are major players in the development of discharge services across the Bristol, North Somerset and South Gloucestershire area (BNSSG). Working with partner organisations in Health and Social Care we are developing plans for a new Integrated Care Bureau which will centralise access to many of the community services in BNSSG which support patient discharge.

University Hospitals Bristol NHS Foundation Trust, North Bristol Trust and Weston Area Health Trust are all  recruiting additional Band 6 Acute Trust Case Managers to join their existing teams  working  within their Integrated Discharge Service. 

The role focusses predominantly on managing highly complex cases. The postholder will be responsible for coordinating patient discharge plans within agreed timescales and standards. It requires the postholder to have or be willing to develop extensive knowledge and skills in managing Continuing Health Care (CHC) issues,  sensitive and challenging situations, homeless patients, patients with learning difficulties or mental health needs and patients with highly complex health needs.  The postholder also needs to confident in all other aspects of discharge planning and take a proactive approach to supporting and educating Trust staff.

You will work closely with colleagues from Social Care and Community Health to ensure a team approach to discharge planning which delivers a high quality experience for patients and families.

The postholder at NBT will be based in the Southmead Hospital. Successful applicants will all be prepared to participate equally in rotas covering Saturdays, Sundays and Bank Holidays.
