An exciting new opportunity has arisen for a caring and forward thinking band 4 Assistant Practitioner/ Nurse Associate to support the Diabetes Specialist Nurses in the delivery of inpatient/ outpatient services. The post will be full time and initially funded for 1 year (fixed term contract or secondment considered) with the potential to become a substantive role in the future.

The successful candidate will be an established band 4 with previous experience in supporting patients with diabetes in a clinical setting. The role will pre dominantly be to educate and support patients (and their carers) to self manage their condition and identify/ overcome barriers which may prevent this.

To achieve this the post holder will need to have excellent communication skills and be sensitive to the challenges experienced by this patient population. A demonstrable ability to teach at an appropriate level to the role will be essential.

Additional duties include supporting the DSNs to promote the governance agenda of the team, audit activity and liaising with pharmaceutical representatives to ensure adequate supplies of equipment are available.

The DSNs will support the post holder to achieve role specific competencies and upon completion would be expected to work with indirect supervision within agreed boundaries of practice.
