Are you a Registered Nurse, Physiotherapist, Occupational Therapist, or Social Worker?

We continue to transform our discharge services and pathways for Patients across North Bristol, South Gloucestershire and North Somerset areas, including Discharge to Assess pathways.

We are looking for a dynamic practitioner to join the case management team and help further shape our services in North Bristol Trust.

The post holder will ensure the co-ordination, discharge planning and safe transfer and discharge of Complex Patients within North Bristol NHS Trust.

You will be expected to provide specialist clinical advice and expertise around discharge needs through comprehensive assessment, planning, implementation and evaluation of individual patient’s discharge plans.

This post will be responsible for ensuring patient focused standards of discharge planning are adopted and followed by all members of the multidisciplinary team. You will also educate and support ward staff with discharge planning principles.

The post holder will work as part of the Integrated Discharge Service, in partnership with the established ward staff, allied health professionals, external partners and social care partners to ensure early information gathering, advising and facilitating care planning to facilitate prompt discharge and moves to alternative care services.

You must be able to demonstrate an assertive and confident approach when advocating on the patient’s behalf, and be a skilled navigator and problem solver. You will work autonomously but also within a wide team.
