As part of an initiative to improve patient flow and the transition from acute to community services, additional therapists are required to support the new Integrated Care Bureau. This is a multi disciplinary/multi agency team covering Bristol/North Somerset/South Gloucestershire, with the aim of further developing and embedding this new approach. We are seeking two part time Occupational Therapists or Physiotherapists to join our acute teams to enable participation at daily navigation meetings, where rehab and care needs are reviewed to facilitate the best pathway for individuals when they leave hospital. Part of the role will also be to clinically manage a caseload of patients within complex care in your professional capacity.

The post holders will be based at Southmead Hospital, which has excellent facilities within the new Brunel building to provide a wide range of services including regional services for neurosurgery, renal medicine, burns and plastics, orthopaedics and major trauma.  

We are seeking enthusiastic individuals, with relevant experience, who are keen to develop their skills and contribute to service development. The ideal candidate will have a good understanding of patient flow and discharge processes, both from acute and community perspectives. Further training and CPD opportunities are available. Regular supervision and support is provided by senior therapists within the team.

There is scope to negotiate the exact number of hours per post. Some weekend working will be required.
