An exciting opportunity has arisen to set up and develop an innovative heart failure service. Our focus is on acute inpatient management, reducing re-admissions and providing world class expert care to our complex patient group.

We are looking for two heart failure clinical nurse specialists who share our passion for improving patient outcomes and experience. You will be a registered nurse with an extensive range of experience at band 6 including managing patients with cardiac conditions and end of life care. You will demonstrate the ability to work both as part of the cardiology multi-disciplinary team and autonomously.

The post holders will develop and implement a range of services, ensuring patients receive the highest standards of clinical care. Acting as a lead specialist you will demonstrate a high level of expertise within the heart failure service. This will involve being a visible ward presence and providing specialist advice, education and support to patients, families and staff.

As one of the heart failure specialist nurses you will be providing autonomous patient reviews in acute wards, leading nurse-led clinics and liaising with community teams. You will assist in gathering and inputting data for the national heart failure audit and using this and other quality initiatives to develop the heart failure service.
